About the role
We are currently seeking an experienced Community Support worker to work with us on a full-time basis. As a Support Worker at 1Heart Care Services, every day you're empowered to make a meaningful difference to people's lives. Duties include but are not limited to:
  • Supporting clients to live in their homes and build connection with the community
  • Enable clients' social connectedness, and provide services such as domestic assistance, personal care, meal preparation, medication assistance, respite care, and transport to clients' medical appointments or any social activities
  • Supporting clients' independence at home
  • Embody a person-centred approach by recognising and promoting the individual's unique goals and preferences
  • Play an important role by contributing to the assessment and care planning of each client as required
  • Effectively communicate with your team, to ensure the best outcomes for each client
  • Develop a plan along with the client to improve their lives and follow it on a daily basis
  • Provide support, information and advice to clients on emotional, financial, recreational, health, housing and other social welfare matters, and evaluate and coordinate the services of welfare and community service agencies.
  • Assessing clients' needs and planning, developing and implementing educational, training and support programs
  • Interviewing clients and assessing the nature and extent of difficulties
  • Monitoring and reporting on the progress of clients
  • Referring clients to agencies that can provide additional help
  • Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services
  • Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
  • Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
  • Supervising offenders on probation and parole
  • Assisting young people to solve social, emotional and financial problems
  • Preparing submissions for funding and resources, and reports to government bodies and other agencies
Essential criteria:
  • Experience working in an aged care facility, in-home, or a community Support Worker
  • Holds relevant qualifications or working towards (Cert III or IV in Individual Support or equivalent)
  • One to two years of work experience in a community setting (Aged care, disability)
  • NDIS Worker Screening Check and Working with Children Check (or willing to obtain)
  • Current Police Check (or willing to obtain)
  • Fully Vaccinated for COVID-19 and Seasonal Influenza
  • Has flexible availability and commitment to an ongoing roster
  • Has caring nature, loves to engage with people, and possess a 'can do' attitude
Preferred but not essential criteria:
  • Experience working with people living with functional or mental disability
  • Current First Aid & CPR Training
  • Driver's license & fully insured car
  • Experience with Manual Handling
  • Experienced with Medication Assistance
  • Understanding of person-centered approach, choice and control, and NDIS practice standards
