Our client is a respected Aboriginal community controlled organisation delivering culturally safe early intervention and prevention programs to support Aboriginal and Torres Strait Islander children, young people and families.
The primary focus of this role is to provide holistic support to Aboriginal and Torres Strait Islander young people aged 12 to 25. Using a trauma informed and person centred approach, the Practitioner supports young people experiencing mental health challenges or seeking support with their overall health and wellbeing.
Support is delivered through tailored outreach, collaborative planning and ongoing engagement, alongside the provision of information, advocacy and referrals to appropriate services.

This role will involve;
- Delivering trauma informed, person centred case management to Aboriginal and Torres Strait Islander young people aged 12 to 25 experiencing mental health or wellbeing challenges.
- Providing outreach support including transporting young people to appointments, supporting access to services, accommodation and community activities that promote healthy living and connection.
- Developing and implementing individual case plans, monitoring progress and coordinating supports to meet each young person’s needs and goals.
- Building strong relationships with families, Elders, community members and external services to ensure culturally safe and holistic support.
- Collaborating with multidisciplinary teams and participating in care team meetings, particularly for young people involved in youth justice, remand or detention settings.
- Maintaining accurate case notes, reports and data through online systems while ensuring compliance with organisational policies and reporting requirements
- Tertiary qualifications in Social Work, Psychology Community Services or related field.
- Atleast 2 years experience implementing case management, group programs and therapeutic interventions.
- Demonstrated experience working with Aboriginal and Torres Strait Islander communities
- Proven ability to build trust and engage young people who may be disengaged from services, including those with complex trauma, mental health challenges, or involvement with youth justice
- Current ACT Driver's License
- Working with Vulnerable People Check and willingness to obtain a National Police Clearance
